If utilization is part of the calculation in the Capitated Payment how do they account for the need being there but services not delivered? For example in HCBS, a person needs the service but provider is short staffed so service isn't delivered but that is not a reflection on the persons need for that service.

We hope that the health plans will work on education with claims. There have been previous problems with the MCEs not being transparent on the case specific billing guidelines as it relates to the way their claims operating systems are configured.

Comment: It is important that the MCEs understand the different services within HCBS and the different agency types. For example a home health agency provides medical care: RN, LPN, home health aides, therapies. A non-medical PSA provides attendant care, homemaker. A PSA cannot provide medical care. MCEs will need to become familiar with what a PSA is as well as the other services within HCBS.

Currently some AAAs and other agencies are working on an Expedited Eligibility pilot program for Medicaid and waiver organized by FSSA and Division of Aging. While it is still a pilot program, can you speak at all on how MCEs will impact that pilot? Will it continue through its current agencies, will MCEs take over, etc? If you're not aware, can you point me to a place to best ask?

Our experience from Ohio is that these plans pay a little over $20 a day LESS than traditional Medicaid AND we have delays in payment. With Medicaid being our largest payer, this will create cash flow issues.
